Three patterns stand out. Abridge is hiring the most, with five San Francisco product leads and a sixth role in New York. OpenAI's single life sciences role carries the highest band. Sierra's band is the widest, which fits a role that is levelled at interview.

What does Abridge ask of its AI product managers?

Abridge turns patient and clinician conversations into clinical notes, and its postings say it is live in 300+ health systems and has powered over 80 million clinical conversations [5]. Fortune reported in June 2026 that Abridge is valued at $5.3 billion, raised a $316 million Series E extension in April 2026, and is co-developing a foundation model for clinical conversations with NVIDIA [6].

The five Abridge postings split into two kinds of work.

Model roles. The Foundational Models and Post-Training lead owns "the product strategy that connects research bets to product outcomes." It asks for working knowledge of fine-tuning, preference optimization, evaluation and inference, and a framework for "when to use a frontier model, an open model, a prompted workflow, or an Abridge-trained model." The Evals lead owns the platform 10+ teams use to measure quality, sets standards for LLM judges and human annotation, and must let teams evaluate a new frontier model "within days of release."

Clinical workflow roles. The Clinical Decision Support and Core Product Experiences leads both ask you to shadow clinicians in exam rooms and work across Epic and other EHR surfaces. CDS adds a build, license or partner decision for evidence sources and a warning against alert fatigue. The Forward Deployed PM goes on site at health systems, leads a small pod, travels about 20% of the time, and has to "hold the line on bespoke requests that don't generalize."

Only Core Product Experiences calls healthcare experience "strongly preferred." The other four list it as a bonus. What they all insist on is a high bar for evidence. The post-training posting says "aggregate model scores can hide consequential failures" and that clinical AI needs "escalation, abstention, and human-review paths."

What does OpenAI's life sciences PM role involve?

OpenAI launched GPT-Rosalind, a model for biology, drug discovery and translational medicine, on 17 April 2026, working with partners including Amgen, Moderna, the Allen Institute and Thermo Fisher Scientific [7]. The Rosalind Life Sciences Product Manager owns Rosalind Workbench, the environment where scientists use those tools, data sources and models.

The posting asks you to prioritise workflows "across discovery biology, verification, and CRO management," to prototype with Codex, Rosalind Workbench and GPT-Rosalind before committing engineering time, and to drive enterprise readiness for pharma and biotech: data protection, admin controls, security and procurement. It also asks for people who "have built model evaluations or benchmarks" and who know "a convincing output is not sufficient proof of scientific validity."

This is the role where domain depth matters most. OpenAI wants "substantive life sciences experience," enough to discuss experimental design and data quality with researchers. It says an advanced degree helps, but equivalent practical experience counts. It is also the only one of the eight that names founder experience as a plus.

What do Sierra and Decagon want from healthcare Agent PMs?

Sierra and Decagon both sell customer facing AI agents, and both now hire PMs who specialise in healthcare. The job is to sit with one large customer and ship the agent that answers its patients or members.

Sierra. The Healthcare Agent Development posting says Sierra's healthcare agents "already reach over 50% of U.S. families." Example projects include agents that answer "what's my co-pay for a primary care visit?", schedule across 30+ clinics, and replace paper intake forms. It asks for 5 to 7+ years, senior PM level, executive messaging and demos, and lists HIPAA knowledge, AI agent experience and some React, TypeScript and Go as pluses. Sierra's healthcare page lists customers including Sutter Health, Novant Health and R1 RCM, plus HIPAA, SOC 2 and HITRUST certifications [8]. In January 2026 Sierra and Stellarus announced a partnership for health plans after a Blue Shield of California voice agent pilot scored 4.4 out of 5 on customer satisfaction [9].

Decagon. The Senior Agent Product Manager, Healthcare role covers "Payer, Provider, Pharmacy, Labs, and Life Sciences" and says you will be "among the first dedicated healthcare Agent PMs." It is the most demanding on domain experience: 6+ years with at least 3 in healthcare, "real fluency in how healthcare actually operates," and shipping under "HIPAA, BAA, PHI minimization, SOC 2 or HITRUST." You own the trust posture: consent, escalation to licensed clinical staff and what an agent may not say. It is also the only role of the eight whose posted pay includes commission. Decagon raised $250 million at a $4.5 billion valuation in January 2026 [10].

Do you need healthcare experience to become an AI PM in healthcare?

Less often than you might think. Every one of the 8 postings mentions healthcare or life sciences background, but only 3 (Decagon, OpenAI and Abridge Core Product) make it a firm ask. The other five, including all four remaining Abridge roles and Sierra, list it under "bonus points" or "even better."

What they do require is proof you can work where mistakes matter. Six of eight ask for regulated or compliance heavy experience. Abridge's Evals role wants "a track record of delivering high quality products in domains where accuracy, reliability, and trust are paramount," which a fintech or safety PM can show. The CDS role asks for "a high bar for accuracy in domains where being wrong has real consequences."
